No person shall in any manner, or upon any pretense, or by any device, directly or indirectly sell, expose for sale, possess with intent to sell, exchange, barter, dispose of or give away any cigarette, electronic vaping devices, or tobacco products to any person, over the counter or through a vending machine, without first obtaining a license from the Village Clerk. The cost of a Cigarette License is $100 per license year (July 1 – June 30).

• CTV-100 Cigarette, Tobacco, and Electronic Vaping Device Retail License Application
• CTV-101 Cigarette, Tobacco, and Electronic Vaping Device License – Individual Questionnaire (one for each individual involved in the business)
• CTV-102 Cigarette, Tobacco, and Electronic Vaping Device – Appointment of Agent
• Pleasant Prairie Police Department Questionnaire (one for each individual involved in the business including proposed agent)
• Copy of Wisconsin Seller’s Permit
